Abstract: Studies prove the efficacy and benefits of exclusive breastfeeding not only for the infant but also for the nursing mother. Faced with the influences of breastfeeding, the previous experiences are related, as well as their entire social context. Thus, this research aims to identify the difficulties and maternal perceptions presented during the breastfeeding of preterm infants assisted in a Neonatal Intensive Care Unit at a reference institution in the South of Minas Gerais. Such an approach is justified by the fact that by recognizing and proposing interventions on such difficulties, the premature newborn will have a guaranteed nutritional support, reducing the possibility of aggravation and early weaning, which contributes to a care based on the principles of humanized and qualified attention to the Preterm NB. To better understand the experiences of the study participants, the qualitative methodology with Bardin content analysis was used, and 13 mothers of premature infants were interviewed in the Neonatal Intensive Care Unit. The interviews were guided by guiding questions related to the mother's feelings regarding the breastfeeding of her premature newborn and to the difficulties encountered during this process. The speeches revealed among the main difficulties, the fear of pain related to nipple fissure and the fear of drying the milk, not being able to nourish the child. The effectiveness of the nursing actions in this process was pointed out as a fundamental factor to solve the doubts and inherent difficulties of this phase, and it is possible for breastfeeding to become a very pleasant and possible act conferred with autonomy and safety.
